Enrollment Procedure

To make the intake process as smooth and efficient as possible, we recommend that you download the following documents, print and fill them out, and bring them with you to your interview.

Children between 6 weeks and 6 yrs.
Child enters program on a trial basis, child and parent must meet after 30-day trial period.
After the 30-day trial basis, the child may be accepted into the Program.

Parents must fill out KIDZ Creative Concepts (KCC) application packet: Child registration form, enrollment form, child health exam record and emergency release form.
